We enjoy seeing new patients, and to make your experience as efficient and positive as possible, here is what you should be prepared to bring with you to your first appointment or consultation with one of our doctors:

• Personal identification
• Insurance Information
• List of medications you may be taking
• Current eyeglasses
• Co-payment for your visit

Also, for your convenience, you can download a copy of the forms all new patients are required to complete prior to their eye exam or consultation: Patient Information & Health History Forms; "Signature on File" form; "Approved Participants of Care" form; "Notice of Privacy" as required by HIPPA; and the "Acknowledgement of Receipt," which you are required to sign once you've read the "Notice of Privacy".  Click here to download the forms (twelve pages total).  Bring the completed forms with you on your first visit.
